Tariffs on cash settlement services for notaries and lawyers

The list of documents to be furnished to the Bank when opening an account for a notary or a lawyer:

• Application for opening a current account (Appendix 5)*, undersigned by a private notary or lawyer;
• A copy of Certificate permitting notary activities performance. This copy is to be certified by the issuing body or a notary, or authenticated by the signature of the Bank's responsible employee;
• A copy of the document evidencing registration of this private notary or lawyer with the Tax Authorities. This copy is to be certified by the issuing body or a notary; or authenticated by the signature of the Bank's responsible employee;
• A copy of the document evidencing registration of this private notary or lawyer with the respective bodies in the Pension Fund of Ukraine. This copy is to be certified by the issuing body or a notary; or authenticated by the signature of the Bank's responsible employee;
• The card bearing the signature samples (Appendix 4)**. The signature samples are to be authenticated by the signature of the Bank's responsible employee or notarised.
• If a private notary or lawyer uses hired labour in his activities, he has to additionally furnish the following:
  A copy of the document evidencing registration of this private notary or lawyer with the respective bodies in the Social Insurance Fund for Industrial Accidents and Occupational Diseases. This copy is to be certified by the issuing body or a notary, or authenticated by the signature of the Bank's responsible employee.***
